My review is intended to provide CAREGIVER'S, seeking longterm or interum care for a loved one, information and insight on Glen Oaks Alzheimers Care Center in Urbandale Iowa. This facility is part of Frontier Management, LLC. (Oregan and Texas) Glen Oaks is a for profit, assisted living care center that claims to cater to individuals with special needs such as those with alzheimers dementia. They offer a beautiful facility, with nice rooms, satisfactory meals, and onsight nurse (LPN or RN) 24/7. Here's our story: (xx) is a beautiful soul. She is a 67 year old female diagnosed three years ago with early onset vascular dementia. As husband (&family), durable and medical POA, we loving cared for our loved one until very recently and until symptoms worsened and we were directed by her physician to seek a higher level of care. On May 12th, 2022 she was assessed by management team at Glen Oaks. They met with us, reviewed medical history and admitted xxx to this facility agreeing to provide aide, assistance and needed care we are no longer able to provide. It is with concern and regret, we feel it necessary to share our story and issues we've had regarding this facility. To be clear, since being admitted to Glen Oaks our loved one has taken No LESS than 25-30 falls, of which 5-7 or more have been direct head hits. (all within a 3-4 week span of time) Granted, falls are to be expected from time to time but history will note that xxx has never before experienced falls to the extent she is now. Herein lies my concerns: Staff at Glen Oaks have contacted me by telephone following each fall incident. However, in most cases the reporting staffer explains that "no harm was done", and " xxx is doing just fine ". yet, at no time to knowledge was the attending physician, nor the nurse practitioner called in to access the situation, or request xrays or seek a CAT scan to eliminate the possibility of head injury. AND yes, at the date of this writing xxxx has at least three visable lacerations, with lumps and bruising over her left eye and the other eye is blackened, swollen and brusied. She complains of body aches and pains which, from what we are told, is managed with tylenol. On June 8th, and following yet another fall incident, the facility's attending physician was alerted by a family member. The doctor voiced concern, and too he validated our assertion that he had not been contacted by Glen Oaks about his patient or falls she has taken. He immediately set a time to visit the facility for examination and assessment of injuries. Family was present for the examination. In addition to examination, doctor ordered labs, urine sample, and imaging. We felt this was a good first step. But then, was taken back when we were told by Glen Oaks staffer xxxx that imaging orders were deemed "unnecessary" by management at the facility and therefore cancelled ---Reason- Unknown. (FYI) A facility care meeting was initiated by family earlier on whereby we met with director of nursing (xxx), and facility director, (xxx). The meeting was cordial and appeared productive. We clearly and firmly voiced concerns about the number of falls, and also aired concerns about medication irregularities, no functional call lights in resident rooms, no maintanance staffer (at the time of this writing), no towels or washcloths in resident restrooms , light bulbs not working and needing to be replaced, and other safety concerns, as well as water seeping into my wife's room from an adjescent shower room with (what was explained to me as) a "leaky toilet". (this went unchecked for 4 days) The odor was horrific. All that being said, the main focus is xxx, and the number of falls taken, and what appears to be a lack of resident monitoring and lack of care provided many facility residents. Our question was as it is now: What are you doing to ensure my wife's safety and wellbeing?? [Many readers will at this point question as to why we did not remove xxx from such a place? The short answer is simple and it's because moving to another facility is Not an easy task. Furthermore, moves are stressful and very trying on individuals with middle to late stage dementia. Lastly, a great deal of money has already been invested with another billing statement sitting on my desk with an amount due approaching $ 9k. Therefore our short term goal is to TRY to reach the "Powers that Be" through reason, logic and yes, exposure with hopes and prayers that we will successfully communicate expectations, and see results through positive change] Immediately after meeting we trusted concerns would surely be considered and addressed. ---- Unfortunately they were not, and following more falls, constant calls to me from Glen Oaks staffers and many sleepless nights for me I responded to yet another call from Glen Oaks at 1am on 6/14. I (husband) drove to the facility and found my wife on the floor in her room with complaints of body pain. It was then that I demanded staffers arrange for transport and xxx was taken by ambulance to the Medical center for observation. The attending physician took my report, conducted an exam and ordered imaging. There's more---Due to all of the above noted concerns documents were requested from Glen Oaks facility director xxx, including information in writing regarding my wifes medical records, a list of current medications, dosages, PRN's, and how often (PRN) meds are being administered. I further requested a copy of a care plan and nurse notes with documentation about falls taken. Here's the deal: an initial request was made on Monday June 6, with a follow up inquiry again to medical staffer xxx. The staffer proceded to tell me "computers are down, but information will be ready and waiting for me in the morning" we arrived at Glen Oaks the following day and was met by xxx again who advised again, "computers are down". At this point we ask to speak with facility director xxx and was told she will call. No call was received. Feeling frustrated, we approached another desk staffer and was advised (per director xxx) that requested paperwork will be available at 3pm on June 7th. we stopped in on June 7, at 2:55pm, and waited for facility director xxx to produce the requested information. At 3:15pm, we approached another staffer and informed her we are scheduled to meet with director xxx to collect paperwork and would like to do so. About that time director xxx appears from her office, she looks directly at us, says nothing and continues to walk down the hall offering no excuse, no apology or explanation. Nothing --- All of this is unacceptable, and while we have remained patient and respectful we are escalating this matter until results are visable and consistant. And should you choose to consider Glen Oaks for your loved one you can be assured facility director xxx will tell a different story. But we will assure you this IS our experience, and all things noted here are true and accurate - Lastly, since reaching out to Corporate Office Executives at Frontier Management, LLC, (no response or action from corporate to date) Glen Oaks management have treated us a bit less than cordial, and often times avoiding --and to date we have yet to receive requested information of which we are entitled and all the while we are troubled, and deeply concerned for the safety and well-being of xxx while under the watch of this facility. Intentions are to keep close watch, and continue to demand what is reasonable. Please do check back as I will post again soon with an update. Thank you for your attention.

This place is a mess and a new fancy remodel will not help turn them around. Staff talks to residents like they are children. All the good staff have pretty much left and I don't blame them. Administrator just sweeps things under the rug or tells you what she thinks you want to hear but never addresses concerns. She takes absolutely no responsibility for anything. I never see her out and about the facility. The times I've been there when she is she is just sitting in her office most of the time with the door shut. She will go to family meetings to make a big show and is always super fake during them. I know for a fact that one time a nurse worked a 21 hour shift! When it was mentioned she just shrugged it off and really didn't address it. Makes me wonder how often this happens and makes me fear for resident's safety. Every time I come into visit at night or weekends the residents are just all positioned around the front lobby area staring at each other or parked in front of a TV. Such a sad place.

This is not the place for your family member if you care about them. I saw residents treated like children, without consideration for who they are as people. Falls and injuries are not properly documented and this has been investigated. Medical orders are not followed. There is a significant amount of turnover among the staff and the management alike. When you report these things to management, the care your family member receives becomes worse. Be very cautious before considering this for-profit facility to care for your family member.
