Your leadership and most staff taken an active & personal interest in the resident's welfare. I like the small and friendly atmosphere.. Amy [Name Redacted] is really doing a good job and her staff seem to value her. I hope she can continue to inspire her staff to care and strive to crate a special community.
Safe environment Friendly staff Current administrator is great. Upgraded carpet in common areas this year - looks nice! Host nice family events Build nice facilities. We really like and appreciate our administrator - Amy!
The meals have greatly improved. Josh & Garrett in the kitchen are wonderful chefs & they do a very good job of interacting with the residents. You can tell the folks especially like Garrett by the way they light up when he speaks to them. I have eaten with mom a few timers recently & it was always delicious.
You do your best to make my mom happy. She's not very cooperative but I'm happy with your efforts.
Hospitality, friendliness of staff. Activities available to residents. Competent staff.. We understand it's a thankless job, but appreciate what the staff does for the low pay and long hours worked!
Each staff member is patient with my mom They are kind and express positive feelings toward and about her. Even though she is unable to actively participate, they include her in activities. All requests that we make to accommodate her changing needs + improve her care are honored and implemented whenever possible. Employ individuals with a lower level of certification + qualifications to do more housekeeping + laundry, + meal services, to free CNAs to focus on resdients - Place keypad locks on the doors of residents who are already unable to open doors or defend themselves from aggressive residents. Staff cannot see everything all the time. - Engage family members + inform them explicitly how they can be helpful. Such information would have been very helpful when we move down, but we had to figure it out on our own. Some families don't seem to be able to do this. Help them.
Home-like atmosphere.. None.
Going to the doctor - transportation. Holiday activities need to be easier for some with chronic pain Much appreciation for the transportation to church/doctor walmart Very friendly people
I like being able to be around a lot of people. It gives me good chances to make friends and connections.
This assisted living is now blessed with a great leader and we get good attention each day. I am too blessed to be stressed!

I like the small size of this facility. No one gets overlooked.. Work hard to create a stable staff.
*Name Removed* is sprucing up things and it's looking better - much better.. Still need communication from *Name Removed* Need to be in closer touch need to hear more.
The food offerings--good variety daily and always tastes good.. Do not feel this is really Assisted Living since the only services she receives (since she has left her independent apartment), is the administration of medicine, showers, and room housekeeping. I do her laundry (Mom's choice). Only activity participation for her is BINGO.
You genuine, care about the neighbors and families. You listen and respond to them concerns.. Keep striving to retain & reward great employees as continuity of care is reflected in a community keep striving to strengthen care levels on weekends.. *Name Removed*, *Name Removed*, *Name Removed* & *Name Removed* and others who have left have given 110% to treat neighbor with dignity. We are very grateful.
General care of the resident
1) Work with residents to stay at assisted living even though they may require a higher level of care. 2) Beautiful facility. 3) Our administrator is kind to the residents and shows concern for their well-being.. 1) Offer sugar free food options; ice cream, jelly, etc. 2) Facility could benefit from regular cleaning of carpet, woodwork, walls, doors, etc. 3) Consider offering classes in different subjects/crafts to residents to keep residents mind & body active. 4) It seems residents are lonely and/or bored on weekends as there are not as many staff or activities offered.. My mother requires little assistance as I assist her with shower, hair, etc, and she washes her own clothes. Should she need more help, I'm not sure you have sufficient staff to respond timely.

Try to please residents keep everyone happy
Friendly most of the time. Helpful. Do extra things to help. More family social events. Generally kind people who work so hard. Seems like everything is falling into place - better, friendlier staff w/ exception of one. Things are mostly humming right along. Better communication than ever.. Be less defensive - we're on the same team. Clean better - more often. I come in to clean up a couple times a week. Often paper, etc. on floor. Laundry could work at getting clothes on hangers, less wrinkled. Wipe off walker, his shoes, spots of spills on shoes, walker, etc. I'm sometimes fearful of complaining - want staff to be helpful and not get on bad side - just some facility carpets need replaced in hallways. Better decor in entry.. We have a nursing/aid staff member who doesn't seem to like us. Have been unfriendly. We have to work hard to get along with her. But that's life.
Yard care. Ours is beautiful all year long.. The food is as artificial tasting. The meat is of poor quality with the tenderness and flavor. Being (yuk!). I am pleases with our administrator. He is conscientious with all he sets out to accomplish, a well - rounded individual. (Most of the time ha!)
Nice meals. Activities director seeks any resident for activities he thinks mom would be interested in (ie. bingo, ceramics, etc). More checking in with my resident to see about bathroom situation (cleanliness) & how are you doing? Sat. & Sundays are ghost towns . No activities - limited staff. . Any time she refuses showers (which are twice a week) I would like a call - I live 3 blocks away.
Small facility so no one gets lost . - DON and core staff very good and caring. - Very good relationship w/ my father. . Reduce poor hires and turnover.
I like being here. Very homelike I'm really happy here.
Provide a comfortable home for me to live in.
Always notify of meal times with reminders.. If I need anything, all I have to do is ask. I don't know the charges because family takes care of it.

The food has greatly improved since chef *Name Removed* arrived. Kudos to *Name Removed* + *Name Removed* also. A good variety of activities if the residents take advantage of them. Seems to be a good group of caregivers right now. Turnover is normal but the ones there since it seem very nice.
Care givers are patient, compassionate, and as attentive as they are able to be under the circumstances. They are very understanding of residents' diminished capacity for proper behavior. Unfortunately there are not enough staff to meet simultaneous needs or to do more than address the most urgent aspects of a given situation before moving on to the next patient.. Staffing level needs to be increased. At times a resident needs significant response, but staff is tied up with other residents. When a resident has a situation that requires two staffers, the remainder of the floor is uncovered. Staff needs to be able to be proactive instead of just responding to presented needs. Same applies to maintenance.. Great staff attitude, but the ratio is inadequate for the required level of care in an Alzheimer's facility.
. This facility has recently had an administrative change and several staff & dietary changes. The dining reading up to this change was not good. It appears to be getting better. Quality of staff seems to be improving

. Care with sitting. Teach resident how to sit, to be time and care; report if necessary. Big risk here.
. More security at entrance and exits - A person to check ID of visitors, check in and out! My mother, who is nearly 97, has lived at Colony Pointe, for 3 years. She is also completely blind in one eye, has very limited sight in the other, and wears hearing aids in both ears. I am sure she is not the only resident with sight and hearing limitations. I think the CNAs, and housekeepers as well, need some training regarding the care for residents with sight and hearing problems, which I would imagine are worsened by memory problems many older residents have. Things we take for granted such as simply washing our hands may not be done as well as needed by a person with limited eyesight. The things they touch, call button, telephone, light switch or water faucets may also need attention. The people who do the laundry for the residents need to be careful to return laundry to appropriate drawers or closet areas. Regarding to the hearing, many times the resident may smile and nod, making the CNA assume they have heard and understood whatever is being said to them and this is often not the case at all. She is very content at Colony Pointe and likes all of the staff. Thank you.
. Work to improve food. I have medicine that is to be taken at 2 PM it is often forgotten. I will need to press my call button and remind a CNA
