. My mother's missed 3 doses of anxiety medication because her dosage was increased from 1x 1 day to 2x 1 day and this adjustment was not made properly to get it refilled on time. My father's rocking chair was not kept clean around the spindles.

The staff is friendly and caring. Great place for an elderly person who can still get around good & has their mind.. The rooms are clean & nice, but a few times I came to visit the bathroom was dirty from the resident using it and wasn't clean. Looked like the poop spots were there for a few days. Wish they would check residents bathrooms each day.

We cannot rave enough over the friendliness & care/concern of the South Pointe staff. Their concern for the physical & emotional well-being of the residents is so evident & heartwarming. We fully realize this is not an easy job. Thank you to all of the wonderful staff! Other things you excel at: Medical assistance, keeping the family informed of serious matters, & creating a home-like setting.. A wall thermostat in resident rooms would be so helpful. The residents have a difficult time reading down to control the HVAC units & have actually fallen trying to do so. Perhaps periodic email 'blasts' about community activities, upcoming events, etc? (Like a newsletter? Something other than the photos on Facebook.)

Respect the resident's individual needs.. Laundry per person - not mixed residents - perhaps on same day each week - unless emergency/illness, Iron on labels on clothing if needed instead of magic marker.

Employees are very friendly and helpful. The phone always gets answered after 2 rings. Food is very good. Love the chef! The staff is very committed and dedicates to taking care of our loves ones. I feel my mother is very well taken care of. I feel very secure with leaving: her in such good care!. The outside grounds needs to be kept up better. I have watered & cleared the fish pond every time I visit my mother. Flowers where planted in the spring but never watered. Branches and leaves - laying on side walk, hard for seniors to walk. To many flower pots (with nothing in them) around the fish & pond. If you can't afford to keep the fish pond, you should have it removed. Just put a water fountain in.. Need more craft projects on Sunday. I really enjoyed working with my mother and the others last week painting the pumpkins. I enjoyed getting to know the others and my mother actually go involved! Great way to meet other, sons and daughters. Suggestion - maybe you could ask the other children if any body would like to be on an I am an activity for Sunday Committee. We could plan a different activity for every Sunday.

After four years of frustration citing the lack of regular transportation, I am pleased that South Pointe now has a van dedicted to Washington, MO and not one that is shared with five other communities. This enables *Name Removed*, (who is a total gem, wonderful asset and true delight), the opportunity to take residents on more meaningful outings as well as doctor appointments. My mother has really benefited with the ability to leave the confines of the facility in the van and embark on external activities. Over the past four and a half years and many, many visits, I along with my mother, have grown close to the fine women of South Pointe, including *Name Removed*, *Name Removed*, *Name Removed*, *Name Removed*, *Name Removed*, *Name Removed* and *Name Removed*. I like their spunk, spirit and genuine affection. I also like that there has not been a lot of staff turnover, thereby deepening relationships, trust and camaraderie.. How about a monthly happy hour? I know of many facilities that offer residents and family members a weekly happy hour with beer and wine or even cocktails. Why not consider that on a monthly basis? First Friday of the month from 5:30-7 pm, so as not to interfere with the 5 p.m. dinner schedule!

I am happy to report that I have seen marked improvement in two crucial areas: transportation and mindful activities. On the last two surveys I voiced grave disappointment in not having regular transportation available to take residents to doctor appointments or outside activities. After all, it's assisted living and if my mother could drive . she would not need to be there. plus none of her relatives lives near. It appears this concern was heard and acted on as Kim and Donna have taken my mother to doctor appointments when there is no church member or family visiting to do so. Also

Friendliness of staff and devotion to resident. I'm very impressed with this facility and the work all the staff does. I'm glad we chose this facility.

Offer a clean and friendly facility for the person to stay at. Feel that the person is taken care of and I don't have to worry as much any more.. I would also like the staff to get the resident out of their room more and go to activities. Especially exercise activities. I feel that my resident doesn't get enough exercise. Also would like the staff to walk them around the building more too. Just to get out for a little bit. My person doesn't like to go on off campus activities so would like them to do more on site with them a couple times a week for at least 30 minutes. In conclusion, I am happy with this facility.
