You all are always looking out for the well being of your clients. Everyone interacts well together. The clients are up beat & joking & are crazy about Misty, the activities director. It's a joy to know our mother is well taken care of physically, mentally & spiritually.

People that work here - caregivers have a huge heart and passion for their patients. Residents should have a lock box or safe in their room for care of valuables.

Concern staff has for residents. Everyone is friendly and caring. Nursing oversight is excellent. It is a warm and comforting place for residents and visitors. You provide a safe and inclusive atmosphere that is an intimate feeling home away from home.. I think it would be nice if you changed the seating every 2 weeks or once a month to 1st people get to know and eat with different people rather than same table assignments in the dining room day after day if the resident agree they want this

Staff is friendly and courteous and very helpful. Provide a variety of activities for residents.. Apparently there is an accounting issue. My mother has Tenncare supplement. I have paid her portion every month as well as any personal hairdresser charges and I have a large outstanding balance that has not been explained to me.

several of the staff seem concerned for the residents

Love *Name Removed* and the regular staff. Sometimes contract staff from outside agency is not up to par. Activities are well planned!. New head nurse seems distant and difficult, at times, to talk with. More approachable would help. Increase pay for caregivers. They are not paid enough for this important job!

Good food, friends + helpers. Love having pets around. Lots of activity, love and care.. Hello! Grandmother is very happy here. Convenient to my home and a good safe place for her. She has made friends and is involved in all the activities. The kindness of *Name Removed*, director sold me on Azalea Court. Professional, yet warm. Staff is always kind and considerate. Dogs and cats are a plus!

Family atmosphere. Pricing. Employees are very much.. Suggest paying caretakers enough so they can pay their bills. many have multiple jobs to make ends meet which ends up making them feel resentful.

Have a place for fellowship with people her age.. Have a place where the patient can lock up personal items, rings, other jewelry etc. Provide transportation for doctor's appts, or etc.

Administration has a caring attitude in response to the resident and her family's needs and concerns.. Craft activities have declined seemingly due to budget concerns. That was an important activity for resident. Staying on top of medical issues, such as continuing sinus problems, to ensure that the issue is not more serious.. Overall experience for several years has been very good, and is certainly greatly appreciated.

Loving the residents!. Speed up the process for getting medicine when it is needed quickly.. I love the staff and how they love the residents! I would love to see their pay increase so they could keep the good ones!

Activities Director does an exceptional job engaging residents. Works to find areas of interest for each resident. Because my mother had poor eyesight, *Name Removed* worked to find 'touch' activities for her.. I am attaching a sampling of notes I kept during a particular time when we were having trouble with my mother's care. A copy was given to *Name Removed*, Executive Director, who never adequately addressed our concerns. On two occasions when my mother experienced bruising on her arms reportedly from a particular resident grabbing her, the Executive Director said she would review security tapes. We never received a response to either incident. NOTES REGARDING EDITH L. BARNETT by ANN BARNETT (daughter) MONDAY MAY 22, 2017 Went to The Arbors to help mom eat dinner. She was already at the table. When the food was put on the table, I could not identify the meat but was told it was a pork patty. It was dark on one side and almost raw on the other. She was also served mashed potatoes and a pickled green bean/corn combo as well as a biscuit that was too hard to eat. Mom would not eat any of it. I came home and fixed her a pimiento cheese sandwich and pears & cottage cheese. She ate it all. Dessert was apple cobbler which she finished. Most of the other residents could not eat the meal so the cook fixed grilled cheese sandwiches for everyone. THURSDAY MAY 25 Received a call from Faith at 10:30 pm tonite saying mom had hit her head. No bruising or knot is visible. Just wanted me to know. (No note was made in her chart about this.) FRIDAY MAY 26 Went by before work at 8:00 am to check on moms head. No caregivers were present in the living room until I had been there 10 minutes. Never saw the nurse. Mom appeared okay but did remember hitting her head when I asked her where it hurt. She rubbed the top of her head, but no marks were visible. Went back to check on mom at 5:15 pm. She was at the table. Dinner was a Reuben sandwich on toasted rye bread. It was too hard for mom to eat. The only other item of food was French fries. The cook fixed mom a grilled cheese sandwich and she ate most of it. SUNDAY- MAY 28 Meaghan called me at 8:00 am to report that mom had been disruptive and aggravating other residentsone of whom grabbed her and opened a skin tear on her arm. She would not let Meaghan bandage it. Meaghan asked that I come help calm mom down so she would eat breakfast and get her arm bandaged. When I got there, mom was in the nurses station and Meaghan was feeding her sausage, oatmeal and scrambled eggs. I could tell she was starved because she couldnt get the food in her mouth fast enough. I took over feeding her so Meaghan could work on her arm. Mom resisted, but we distracted her with eating. We discussed moms continued agitation, lack of sleep, and not eating. I expressed concern with the food, especially after Mondays experience with the pork patty and hard biscuit. Also asked Meaghan about turnover in caregivers. Guess its just part of the industry that they are not paid well. Meaghan mentioned that some of the agency personnel that are being sent to work at The Arbors are not qualified and that it requires a lot of her time [Truncated]
