Just moved to Arizona from the state of Hawaii and Peoria Post Acute and Rehabilitation has become my family away from home. My new “Ohana” across the sea. The team at Peoria Post Acute and Rehabilitation has an enthusiastic energy that vibrates positivity, team work and genuine wellness. Patients here are treated with respect and compassion and staff truly value each resident’s health, safety and happiness. I really love that the leadership team encourage staff to celebrate each other’s achievements and motivate staff to keep striving to be the best!

Reading some of these low rating reviews, I can attest to their concerns. While my loved one was in their care, we witnessed potential falsification of records (staff reporting they administered a patients non-narcotic maintenance medications 2 nights in a row, when the medication in daily sealed bubble pouches were still accounted for). Our loved one stated they were practically accused of lying despite being of sound mind regarding this. I believed my loved one on this as on the first day I was visiting, I had a cloth mask, asked staff at nurses station for an acceptable mask after staff in the hall commented, and I was personally accused of being dishonest about not already being given an acceptable mask. They asked if I had actually even checked in and spoken with front desk. I was clearly checked in with a current days visitor badge, and was at the nurses station in the center of the facility. When I pointed this out, I was asked again "did you actually check-in with someone at the front desk?"... again, I had a current days visitor badge and was in the middle of the facility, I surely would hope so. During discharge we were given another patients extremely detailed records. To quote the facilities earlier response to a review, "I am alarmed" at the clear HIPPA violation we witnessed. Additionally, Food concerns are real and documented. I would actually worry for a patient that is not of sound mind that they may not always be administered their medications despite records saying the contrary. Don''t get me wrong, there are SOME staff members that did understand these concerns and were genuinely caring, however that does not change the above concerns witnessed.

I’ve worked at Peoria Post Acute for about 7 years and I have seen the changes and improvements over the years, and I myself have grown a lot here professionally. This place isn’t perfect, no place is, but there’s a thousand reasons why it’s a great one! Culture is great here, people get along and you will get that vibe here as you walk the halls! And last but not the least, has the best rehabilitation team lead by Tina Bond :) I hope you find this review helpful and hope you will have a great stay at Peoria!

When we left with her and inquired about a doctor discharging her, we were told ‘just take her. She shouldn’t be here anyways.’ If your loved one has dementia or Alzheimers, search for another SNF.
